Terms & Conditions

Dr Carlton-Conway practices independently when offering private allergy appointments, using available clinic space at St Albans Consulting Rooms. Payment for your allergy appointment is required to be paid in advance. We will usually, in advance, take payment for an allergy appointment plus the additional cost of skin prick testing unless this is not likely to be needed. Up to date costs can be found on our website www.hertsallergy.com/prices. The advance payment for the appointment will need to be paid at the time of booking and in order to secure confirmation of your appointment. 

 

Where skin prick testing is not required following the consultation, the fee for the skin prick testing will be reimbursed back to you within 10 working days.

 

Any additional costs incurred at the allergy appointment that have not been paid in advance, will become immediately due following the appointment and must be paid within 2 working days.

 

Appropriate allergy testing will be considered and arranged after an in depth consultation. Please note that allergy testing is not always clinically indicated or required following a consultation. The need for allergy testing is at the discretion of Dr Carlton-Conway. Although the fee for skin prick test will be refunded in this scenario, the cost of the consultation is not able to be refunded.

 

Where any additional tests such as blood tests are required to further investigate your allergies we use The Doctors Laboratory (TDL) Service and also Spire Hospital in Harpenden.  For TDL, this will require your attendance at the TDL patient reception in London for these to be taken. There are additional costs that you will pay for blood tests directly to TDL which are:  the cost of the blood tests themselves,  the cost of phlebotomy by TDL, which is currently £60 and may vary from time to time and if requesting an invoice , a processing charge which is currently £28.50 but may change from time to time. For Spire Harpenden, this will also require attendance. You will pay Spire Direct a phlebotomy fee of £30 will apply which may vary from time to time as well as the cost of the blood tests. 

 

Following blood tests, we will write to you with the results. In the event that a consultation is required to discuss the results, we will suggest that you book a follow up consultation to discuss these.

 

If you wish to cancel your appointment, please give a minimum of 48 hours notice by emailing hertsallergy@gmail.com. If your appointment is on a Monday or Tuesday, you will need to give notice by 12pm by the Friday before.  Non attendance, late arrivals or cancellation within the above time frame will incur a £130 charge. Late arrivals more than 10 minutes after the designated consultation time will be not be seen and will incur a £130 cancellation charge. Any balance due will be reimbursed back to you within 10 working days.

 

 

Your Personal Data 

When you attend an appointment, notes will be held securely by Dr Daniel Carlton-Conway. A copy will also be sent with your consent to your GP surgery and other relevant clinicians/parties. We will send your clinic letter, password protected, via an email to the email address that you entered on the new patient form. Any handwritten notes made during the consultation will be shredded  as these will be transcribed into the clinic letter. Data may also be shared with relevant third parties, such as TDL for investigation purposes such as blood tests and administration, St Albans Rooms for appointments, and any assistants of Herts Allergy.
